Advance course preparation for the Train-the Trainer course requires attendees to:

 Complete the OSHA Medical Questionnaire form found at this link: 

  1. :  OSHA Medical Questionnaire Form 

      2.  Secured the signature of a physician or other licensed healthcare professional on the Medical Recommendation form and bring it to the training  Click Here for the Medical Recommendation Form

     3.  Each participant should bring their Fit Test Kit and N95 mask with them to the training.  The test kit should be similar to the one found at this link:  N95 Fit Test Kit

If you have a scheduled committee meeting, please use the call in method for attending virtually.  The information on how to do this will be sent in a reminder email with the appropriate "Go-to-Meeting link prior to your scheduled meeting.  Please don't hesitate to call the PEMS office if you have any questions.
